IMPORTANT CHANGES TO YOUR PERSONAL DATA

The Practice is required to supply your personal and confidential medical information to the Health and Social Care Information Centre (HSCIC). This information will be taken from the practice and includes your postcode and NHS number but not your name and sent to a secure system where it can be linked with other health information.

The practice has to allow the HSCIC to extract this information.

 

From January 2014, every household in England will be sent an information leaflet explaining more about data extraction and what the information will be used for.

If you do not want information that identifies you to be shared outside the Practice please send us written confirmation and we will note this on your medical records. This note will prevent your confidential information from being used other than in special circumstances allowed by law, such as when there is a public health emergency.

Online Repeat Prescriptions

IMPORTANT MESSAGE: We will be changing the way you order your repeat prescriptions on 1st April 2014 to on-line via the Patient Access facility. To register with this service go to www.patient.co.uk/access or ask at our reception for details. This facility will replace the email link currently in use.
